분사구문에서 주절의 주어와 분사구문의 주어가 일치해야 하는 것이 원칙입니다. 하지만 실제로 원어민들은 이 원칙을 지키지 않고 분사구문을 쓰는 경우가 많습니다. 문법적으로 옳지 않은 쓰임이고 좋지 않은 문장으로 평가합니다만, 그렇게 쓰는 사람들이 생각보다 많으니, 문법 문제를 푸는 경우가 아니라면 문맥에 따라 이해하시는 게 좋습니다.
질문하신 내용도 마찬가지입니다. 문맥상 multiple fracture 때문에 치료가 어렵고, 거북이가 usual walking pace를 유지한다는 말이겠죠. 문맥에 맞게 이해하시면 됩니다.
하지만 두 문장 모두 좋은 문장이라고 할 수는 없습니다.
